Registration Materials
Welcome to our practice. Our focus at San Diego Sexual Medicine is on YOU, the patient. Members of both the professional and administrative staff of SDSM consistently strive to create an environment where you may experience a sense of trust and feelings of respect. We are committed to the highest level of integrity and responsibility, working to meet the needs of our patients and their families, as we constantly aim for excellence in patient care and practice management.
Registration materials include forms to be completed regarding your medical and sexual history and various assessment tools that we would like you to thoughtfully complete. Because sexual health problems encompass biological, psychological and social issues, completing these questionnaires gives us information that will help in discussing, diagnosing and treating your sexual health concerns. If you live out of town, we would appreciate your sending all the materials to us ahead of time.
